Notes disappear also message problem

How do you keep notes from deleting themselves.
Also there seems to be a problem once in a while when opening messages from the lock screen. The phone will just reset itself instead of opening the message.
I've tried doing resets but that doesn't work. Oh and this is on an iphone 5.
